ACTS Senior Care Navigation is run by Licensed Nurses who understand the medical, emotional, and safety needs of aging adults. Our nurse-led structure ensures that every recommendation is rooted in health knowledge, care-level appropriateness, and real-world senior care experience.

To ensure well-rounded and expert guidance, our team collaborates closely with:

• Licensed Social Workers – psychosocial support, resources, and discharge planning
• PharmD (Doctor of Pharmacy) consultants – medication safety, drug interactions, and chronic condition considerations
• Nurse Practitioners (NPs) – clinical insight on care levels, health stability, and long-term planning

This multidisciplinary support system allows us to provide the most reliable, informed, and holistic senior navigation experience available.
